The evidence is clear: AF screening must start earlier, and the recommended age of 55 is not arbitrary but a strategic response to the unique risk profile of Indigenous Australians. This is not merely a matter of technology; it's about systems of care that are responsive to the needs of Indigenous communities. Portable single-lead ECG devices have proven feasible and acceptable in Aboriginal Community Controlled Health Services, enabling opportunistic screening and early detection of AF. However, the journey doesn't end with detection. A structured clinical review is essential, encompassing diagnosis confirmation, stroke risk assessment, anticoagulation management, symptom control, and addressing co-existing cardiovascular risks. This requires clear referral routes, timely reviews, culturally safe communication, shared decision-making support, and adequate resourcing for Aboriginal Community Controlled Health Organisations and primary care teams. The call to action is clear: national leadership is needed to update AF screening recommendations, reflecting the evidence and the unique challenges faced by Indigenous Australians. The benefits of earlier screening are profound: preventing disability, preserving independence, and saving lives. It's a practical, evidence-based approach that can close the cardiovascular gap and ensure that Indigenous Australians receive the care they deserve.
